Purpose:
This grant aims to provide funding for tablets and tablet-related accessories to Skilled Nursing Facilities (SNF) and associations representing these facilities. The tablets will be utilized for virtual telehealth appointments and family visits for SNF residents.
Description:
Skilled Nursing Facilities can apply for funding through this grant opportunity to acquire tablets and tablet-related accessories. It is mandatory for facilities to confirm that the tablets will be used to facilitate virtual telehealth appointments or family visits for residents. The facilities must also be certified by the Centers for Medicare and Medicaid Services (CMS). While the general funding request limit is $3,000.00, requests exceeding this amount will be considered but will undergo review by CDPH and CMS.
Applicants are required to use the template provided on the CDPH website; cover letters are not necessary.

Applicants can be based anywhere in the United States; however, the recipient of the funding must be a certified Skilled Nursing Facility located in California.
